IN ADDITION TO THE GENERAL NOTARIZATION GUIDELINES

  • Patients must be coherent, fully able to comprehend the nature of the documents they are signing, and able to communicate verbally or by writing.

  • Ensure that the patients have not been given any pain or anxiety medication within the last 8 hours. Documents cannot be notarized while patients are under the influence of these drugs.

  • With new wills, changes to existing wills, or in the case of end-of-life care, the patient, their witnesses, and/or family and friends, must agree to having the signing filmed. This video will be shared with all signers following the completion of the documents. This serves as further proof of a non-coerced and full conscious signing. This also provides the ability for patients who may not have the use of their hands, to acknowledge verbally, and have a signer in their place.
